About the Role

As a Community Resource Navigator under the Case Management Manager's guidance, you will engage in focused outreach to members facing social determinants of health (SDOH) challenges. Your work includes evaluating psychosocial needs and facilitating access to vital services overcoming obstacles like housing instability, food insecurity, transportation issues, and access to care. Acting as a "Service Connector," you will bridge communication between members, their families, the health plan, and community resources to promote equitable health outcomes.

Key Responsibilities

  • Identify and reach out to members experiencing potential social determinants of health barriers, conducting thorough psychosocial assessments.
  • Inform members and their families about eligible benefits, programs, and community services available to them.
  • Organize and manage referrals to both internal initiatives and external community agencies.
  • Maintain detailed records of interactions and interventions within the care management system, utilizing standardized templates and questionnaires.
  • Locate and establish connections with community-based service providers to support members effectively.
  • Create internal referrals to clinical personnel, such as nurse health coaches and social workers, as appropriate.

Qualifications and Skills
