Care Management Coordinator

Role Overview

This position involves coordinating and contributing to Care Management services for family members and caregivers associated with the 24/7 Telepsychiatry team. The team includes psychiatrists, social workers, and coordinators, with the goal to facilitate timely and efficient care delivery to patients.

Key Responsibilities

• Apply patient-centered motivational interviewing methods to establish rapport and encourage health improvements.
• Support primary care teams through panel management to reduce patient loss, noncompliance, or disconnection from care.
• Develop, maintain, and adjust care plans tailored for high-risk patients, addressing medical and social obstacles to care access.
• Serve as a professional intermediary among hospitals, primary care providers, specialists, and community resources to ensure coordinated patient care.
• Identify and monitor special populations, including high-risk patients and those needing preventive or chronic care services.
• Track patients recently discharged from inpatient or emergency services.
• Employ team communication strategies to ensure completion of referrals, hospital follow-ups, and other care plan components.
• Conduct outreach activities across primary care sites, patient homes, hospitals, and neighborhoods.

Qualifications and Experience

• Minimum of a High School Diploma or equivalent; Bachelor's Degree preferred.
• 1 to 3 years of relevant healthcare experience required.
