About the Role
The Care Coordinator plays a pivotal role in facilitating high-quality care coordination by leveraging the Unite Us platform. This role involves direct engagement with clients needing services and the partners providing resources, helping assess client needs related to social determinants of health, connecting clients to appropriate assistance, and ensuring follow-through on services provided. Additionally, the Care Coordinator supports network health monitoring by analyzing referral patterns, partner responsiveness, and identifying barriers that affect care coordination across healthcare and community organizations.
Key Responsibilities
- Conduct client intake, needs assessments, and proactive outreach using data-driven insights to identify social and health-related needs.
- Connect individuals with appropriate service providers based on eligibility, location, and client preferences.
- Monitor referrals to ensure timely actions and successful outcomes, including follow-ups with both clients and providers.
- Build and nurture relationships with community-based organizations (CBOs), offering feedback to maintain network standards and facilitating accurate referral data management.
- Provide insights on referral trends and barriers to leadership to enhance network performance and client outcomes.
- Maintain expertise in the Unite Us platform and continually expand knowledge of community resources and services.
- Operate independently with high efficiency and consistently exceed performance goals and KPIs related to client service, referral management, and collaboration.
- Participate in cross-functional projects and support teammates to ensure contractual obligations and team targets are met.
Qualifications and Experience
- No strict education requirement; relevant experience and skills prioritized.
- Board-certified case manager (CCM®) certification or the ability to obtain it is advantageous.
- Bilingual Spanish is a desirable plus.
- Preferred experience includes care coordination, case management, social work, or roles involving support navigating social services such as housing, transportation, health, employment, benefits, and financial assistance.
- Experience in monitoring referrals, supporting service linkages, and identifying service gaps or barriers is preferred.
- Experience analyzing network activity and referral performance indicators to support care coordination outcomes is beneficial.
Compensation and Working Conditions
- This is a full-time remote position based in the U.S. with up to 10% travel possible.
- The annual salary range is $50,000 to $52,000, depending on skills, experience, and location.
- Employment is contingent upon passing a background check covering criminal history, employment, and education verification.
