Position Overview

A prominent hospital group in Abu Dhabi is recruiting a skilled Case Manager with a solid nursing background and extensive case management expertise. The role requires fluency in both Arabic and English and a preference for candidates experienced in Long-Term Care, rehabilitation, or chronic disease management. The Case Manager will be integral in planning, coordinating, and optimizing patient care to ensure seamless transitions and resource efficiency throughout the care continuum.

Key Responsibilities

  • Oversee and coordinate patient care plans throughout their treatment journey.
  • Perform thorough patient evaluations to determine clinical, psychosocial, and discharge requirements.
  • Collaborate closely with healthcare professionals, including physicians, nurses, and therapists, to provide comprehensive care.
  • Track patient progress and facilitate prompt interventions to improve clinical results.
  • Manage discharge processes, including referrals and transfers to rehabilitation, long-term care, home healthcare, or community services.
  • Serve as a communication conduit among patients, families, healthcare teams, and external organizations.
  • Ensure efficient healthcare resource utilization while maintaining high-quality care standards.
  • Maintain meticulous case management records and reporting.
  • Support adherence to regulatory, accreditation, and hospital quality mandates.
  • Engage actively in multidisciplinary team rounds and patient care meetings.

Qualifications and Requirements

  • Possession of a Bachelor's Degree in Nursing from an accredited institute.
  • Valid UAE Nursing License (DOH, DHA, or MOH) or eligibility for licensing.
  • A minimum of 3 to 5 years of clinical nursing experience, with at least 2 years dedicated to case management.
  • Experience in Long-Term Care, Rehabilitation, Chronic Care, or Care Coordination is strongly preferred.
  • Fluency in Arabic and English, both spoken and written.
  • Comprehensive understanding of discharge planning, utilization management, and coordination of patient care.
  • Strong communication, interpersonal, and problem-solving abilities.
  • Capability to operate effectively within multidisciplinary healthcare settings.
  • Familiarity and proficiency with electronic medical records and healthcare information systems.
