About the Role

We are seeking an experienced Executive (Social Worker) to join our Residential Case Management Unit at PERTAPIS Senior Citizen Fellowship Home in Singapore. The role involves ensuring resident compliance with rules, formulating individualized care plans, and overseeing clinical intervention services such as counselling. You will also assist in maintaining security, collaborating with external agencies, preparing detailed reports, managing case files, and facilitating resident reintegration efforts.

Key Responsibilities

  • Develop and implement Individual Care Plans tailored for each resident.
  • Manage the Centre's clinical intervention services including counselling and care support.
  • Assist in maintaining the safety and custody of residents within the Centre.

  • Ensure the adequacy of security protocols and measures at the Centre.
  • Support the Deputy Head of Home by preparing reports on serious incidents concerning staff and residents for the Management Committee.

  • Conduct assessments of potential clients for admission suitability.
  • Coordinate resident suitability assessments and facilitate their discharge when necessary.

  • Guide the application and renewal processes for Medical Fee Exemption Cards and Means Testing for residents.
  • Organize and conduct group counselling, family sessions, and one-to-one sessions with thorough documentation.
  • Arrange home and hospital visits alongside other pertinent duties.
  • Work closely with residents and families to support possible reintegration.
  • Provide staff updates on case developments and keep families informed of significant happenings.
  • Contribute to the Centre’s development and support operational activities.
  • Execute any additional tasks as assigned by the Head of the Home, Licensee, or Governing Board.
  • File Police reports when necessary.

Candidate Requirements

  • Possess a degree in Social Work.
  • Have one to two years of experience in casework and counselling.
  • Demonstrate strong case management and counselling abilities.
  • Certification in mandatory counselling cases is advantageous.
  • Exhibit excellent interpersonal and communication skills.
  • Be detail-oriented and a collaborative team player.
  • Maintain high accuracy and efficiently manage follow-ups.

Working Hours and Location

Work schedule: Monday to Friday, 9 AM to 6 PM, five days per week.
