Job description
Role Overview
The Social Work Coordinator plays a pivotal role in delivering financial assistance and discharge planning support to patients. They collaborate closely with Medical Social Workers by assisting in casework and contributing to the effective administration and operation of the Social Work Department.
Key Responsibilities
- Handle administrative and operational tasks vital for the smooth functioning of the department, including managing the service counter and associated duties.
- Engage directly with patients and their families, conducting intake interviews and assessing financial needs to offer appropriate support, while aiding referrals to support discharge planning within selected care areas.
- Assist Medical Social Workers in managing cases by providing translation services, patient escorting, and conducting home visits.
- Take part in enhancing work processes and participating in quality improvement initiatives.
- Collaborate with internal hospital departments and external organizations to optimize patient care and experience.
- Support departmental programmes and special projects as needed.
Qualifications and Skills
- This position reports to a Medical Social Worker and coordinates with the Team Leader for Social Work Coordinators.
- A diploma in Social Work is the minimum academic requirement.
- Strong interpersonal abilities and problem-solving skills are essential.
- Proficiency in IT skills is required to effectively fulfill job responsibilities.
