About the Role
This one-year fixed term full-time position, based in Limerick City, involves working within the Housing First Case Management Service to implement the Housing First approach for individuals experiencing long-term homelessness.
Key Responsibilities
- Adhere to Housing First principles promoting client autonomy, choice, and independence.
- Provide a supportive, safe, and welcoming environment for people who are homeless or at risk of homelessness.
- Establish and maintain trusting professional relationships to help clients achieve stability and sustain independent living.
- Conduct comprehensive assessments, develop care and support plans utilizing Housing First case management systems and measurement tools.
- Assist clients in obtaining and retaining appropriate housing, welfare benefits, and other accommodation options.
- Support tenants in maintaining successful tenancies through life skills development, rent management, reporting repairs, and community integration.
- Forge strong working relationships with social housing providers, private landlords, and relevant homelessness and accommodation services.
- Advocate and accompany clients to healthcare, counseling, legal, social services appointments as needed.
- Collaborate effectively with multidisciplinary teams and partner agencies from statutory, voluntary, and community sectors.
- Offer advice and guidance to prevent homelessness and facilitate client access to education, training, employment, and community opportunities.
- Manage an active caseload with accurate documentation, including case notes, assessments, records, and database updates using Focus Ireland systems and PASS.
- Participate in supervision, team meetings, training, external meetings, and contribute to service improvement and best practices.
- Demonstrate flexibility to meet service demands, uphold health and safety standards, support volunteers or students, and perform other duties as assigned.
Qualifications and Experience
- Degree level third-level qualification in social care-related fields such as youth work, social studies, addiction, psychology, or equivalent.
- OR registration as a Social Care Worker with CORU.
- Experience working with individuals currently or previously homeless.
- Experience supporting individuals with dual diagnoses.
- Proven ability to work within a team environment.
- Competency in report writing.
- Experience coordinating with other agencies and organizations.
- Understanding of homelessness and associated issues is desirable.
- Holding a full driver’s license is preferable.
