Overview
As a Care Manager at Dovida Australia, located in Margate, Queensland, you will play a pivotal role in assisting clients to live comfortably at home through high-quality, person-centred care. This position goes beyond mere coordination to encompass connections, strategic planning, reablement efforts, and making a meaningful impact. You will drive client-focused outcomes and help expand Dovida's relationship-based care services across Brisbane North and Caboolture by building trust and identifying unmet needs.
Collaboration with our enthusiastic office team and national network is key, with the flexibility to work remotely when travelling and in the office as needed.
Key Responsibilities
- Establish and nurture strong, trust-based relationships with clients and their families.
- Manage client onboarding, conduct reviews, and coordinate ongoing care centered on wellness and reablement principles.
- Create and formalise personalised care plans and service agreements tailored to individual needs.
- Proactively promote Dovida’s services to deliver excellent support and promote business growth.
- Provide continuous feedback and collaborate effectively with the Caregiver team.
- Maintain thorough documentation of interactions and ensure communication among all stakeholders.
Candidate Profile
- Previous experience in case management, preferably within aged care or Home Care Packages/Support at Home.
- Exceptional ability to build relationships and communicate effectively.
- Professional and confident engagement with clients alongside service promotion skills.
- Competent with Microsoft Office Suite, including Outlook, Excel, and Word.
- Highly organised with strong skills in time management and documentation.
- Possession of a valid Australian Driver’s Licence and readiness to travel within Brisbane North and Moreton Bay regions.
- A genuine passion for supporting older Australians to maintain independence and dignity.
