Role Overview
The Operations Coordinator for the GUIDE program will oversee and support a vast nationwide network of Residential Care Communities (RCCs), respite care vendors, and franchise locations aligned with the GUIDE dementia care model. This role involves acting as the main operational liaison to partners, managing referral processes, and ensuring smooth daily operations to uphold high-quality, patient-focused care.
Close collaboration with cross-functional internal teams—including Care Navigation, Clinical, Finance, and IT—is essential for supporting operational execution, data tracking, and identifying improvements to enhance partner responsiveness, workflow effectiveness, and caregiver satisfaction.
Primary Responsibilities
- Source and assess potential RCC, respite care, and franchise partners to broaden geographic reach and improve patient access.
- Coordinate partner onboarding by gathering all necessary credentials, compliance documentation, and agreements.
- Maintain up-to-date records of partners, service areas, and operational information in internal databases and CMS portals.
- Monitor contract timelines including renewals and amendments to ensure continuous service provision.
- Serve as principal daily contact for partners to address routine questions and support needs.
- Manage daily referral workflows with clear communication and rapid escalation of urgent or complex issues.
- Maintain engagement through regular outreach and monitor partner service quality and performance.
- Work collaboratively with Care Navigation and partners to address and resolve operational barriers promptly.
- Document all partner communications, issues, and follow-up measures accurately.
- Coordinate with Finance and Technical teams to streamline referral and billing processes.
- Develop, update, and maintain standard operating procedures, workflow guides, and escalation processes.
- Identify repetitive operational challenges and propose actionable improvements to enhance efficiency and patient care.
- Assist in executing new operational initiatives and rolling out system enhancements across teams.
- Monitor key metrics such as referral volume, turnaround time, utilization, and partner responsiveness.
- Prepare and disseminate periodic performance reports and dashboards for leadership review.
- Regularly audit partner, operational, and billing data to guarantee accuracy and compliance.
Qualifications
- At least 2-3 years of relevant experience in operations, healthcare operations, care coordination, vendor support, or network management.
- Experience collaborating with external partners, vendors, or healthcare/social service providers is preferred.
- Exceptional attention to detail with proficiency in managing multiple operational tasks simultaneously.
- Comfortable working with spreadsheets (Excel/Google Sheets), dashboards, and software portals.
- Strong communication skills, both written and verbal.
- Demonstrated ability to work cross-functionally and independently in a fully remote work setting.
Preferred Experience
- Knowledge or exposure to respite care, residential or skilled nursing facilities, home health, or caregiving services.
- Basic understanding of contract management, administrative compliance, or credentialing procedures.

This is a fully remote position within the United States, with necessary home office technology provided. Occasional travel may be required for team gatherings or professional events.
