Role Overview
We are looking for a compassionate, well-organized, and proactive Case Manager to oversee NDIS and aged care participants within the South-West Sydney region (Liverpool / Fairfield West). This role involves acting as the key liaison for participants, their families, and care providers to coordinate individualized care plans and monitor support services. The role also demands compliance management under NDIS and Aged Care Quality Standards and nurturing community partnerships.
Primary Responsibilities
- Conduct intake assessments including initial client evaluations, onboarding procedures, and risk assessments for NDIS participants and Home Care Package (HCP) clients.
- Create, execute, and update personalized Care Plans and Goal Action Plans focusing on participant preferences and requirements.
- Manage an active caseload, consistently monitoring care delivery and ensuring timely reviews of client plans.
- Organize and coordinate schedules for support staff, clinical experts, and allied health professionals to ensure seamless service provision.
- Handle day-to-day scheduling adjustments, resolve service issues promptly, and manage participant concerns effectively.
- Document all necessary case information accurately and promptly within client management systems, including shift notes, care plans, and incident reports.
- Maintain rigorous compliance with the NDIS Quality and Safeguards framework, Aged Care Quality Standards, and privacy laws.
- Review monthly budgets, service agreements, and statements to maintain alignment between client needs and allocated funding.
- Serve as the primary contact for families, Support Coordinators, Plan Managers, healthcare providers, and hospital discharge planners.
- Represent CareLinc actively within Liverpool and Fairfield communities, promoting service engagement and partnerships.
Required Qualifications and Experience
- Possession of a Certificate IV or Diploma in Community Services, Individual Support, Case Management, or a closely related discipline, or equivalent practical experience.
- At least 1 to 2 years' experience managing NDIS cases, Support Coordination, or Home Care Package case management.
- Proficient user of NDIS portals, myplace, or similar client management software such as ShiftCare.
- Strong verbal and written communication abilities in English, with inclusivity for diverse multicultural backgrounds; bilingual skills in Arabic, Vietnamese, or Spanish are highly valued.
- Compliance with certifications including the NDIS Worker Screening Check (or readiness to obtain), current Working with Children Check, recent National Police Check (within six months), valid Australian Driver's Licence, and access to a reliable vehicle.
