About the Role

Wider Circle is seeking a Care Navigator to join our Care Delivery Operations team. This entry-level position involves engaging with patients primarily over the phone, occasionally in person, to conduct health assessments and intake interviews. You will establish trust quickly, inquire about factors influencing health such as nutrition, transportation, and housing, while meticulously documenting each interaction. This role offers a foundational step toward advancing into a Care Advocate position.

Company Overview

Wider Circle collaborates with health plans and providers nationwide to administer community health initiatives. We foster supportive networks among health plan members and neighbors to encourage healthier lifestyles. Our method, proven through published research, enhances member trust, improves experience, and reduces avoidable hospital visits. Our programs are active in tens of thousands of communities across the country.

Key Responsibilities

  • Conduct Health Screenings and Intake Calls: Make calls to patients following a standardized questionnaire, building rapport during 15-20 minute sessions to collect accurate health information.
  • Manage Scheduling and Follow-Up: Directly book intake appointments using Healthie platform; maintain continuity by scheduling incomplete intakes yourself and confirm appointments to maximize attendance.
  • Link Patients to Appropriate Resources: Use open-ended questions to assess patient needs and schedule necessary follow-up visits with doctors or Community Health Workers.
  • Maintain Accurate Documentation: Input patient data live during calls, review notes thoroughly, complete all documentation by shift end, and comply with HIPAA regulations including obtaining patient consent.
  • Achieve Performance Goals: Organize your daily agenda to consistently meet targets, including maintaining at least a 60% show rate for scheduled intakes and allowing time to review notes carefully between appointments.

Candidate Requirements

  • Minimum Education: High school diploma or equivalent; degrees in Human Services, Public Health, or Behavioral Health are advantageous.
  • Bilingual Ability: Strong preference for fluency in English and Spanish or another language.
  • Relevant Experience: Experience in healthcare, social services, community outreach, or with care platforms like Healthie is helpful but not mandatory. Comfort communicating with diverse patients via telephone is essential. No prior intake experience needed as full training is provided.
  • Technology Proficiency: Ability to efficiently navigate web-based applications and adapt to evolving software environments.
  • Independent Work Capability: Effective self-management to meet goals, organize documentation, and work remotely with minimal oversight.
  • Training Commitment: Completion of comprehensive training involving call simulations, documentation practices, and role play, requiring an 80% passing score on the final exam.
  • Background Screening: Successful completion of a drug test and background check prior to employment.

Physical Demands

  • Ability to sit, stand, and walk regularly for patient communication and task management.
  • Good vision and hearing to effectively engage in phone and face-to-face interactions and respond to detailed inquiries.
  • Competence in using a computer to record health information accurately while maintaining conversation focus.
  • Access to stable internet and a private, quiet workspace suitable for remote calls.

Reasonable accommodations will be provided for disabled individuals capable of fulfilling these essential functions.

Compensation and Benefits

  • Hourly wage range of $17 to $19.
  • Opportunities for performance-based bonuses.
  • Defined career progression to the Care Advocate role, including Community Health Worker training, enhanced pay, and additional responsibilities.
  • Comprehensive health insurance including medical, dental, and vision coverage.
  • 401(k) retirement savings plan.
  • Paid time off.
  • Employee Assistance Program.
  • Flexible Spending Accounts for health care and dependent care.
