Role overview
This position supports a home health organization focused on delivering compassionate care in patients’ homes. The company is a long-established, Medicare-certified provider in Florida, and this role is part of a recognized team that helps keep documentation accurate, complete, and compliant.
What you will do
- Accompany care coordinators to referral partner offices, build working relationships, understand each account’s referral flow, and help maintain the preferred delivery process.
- Visit referral partner locations on a regular basis to secure signatures for pending or missing orders.
- Coordinate with referral partners to collect outstanding paperwork when a care coordinator is no longer assigned to the agency or branch.
- Check that physician details on the profile page are accurate and investigate and fix any errors.
- Submit Smartsheet requests when a care coordinator’s referral sources need updating, such as physicians, facilities, hospitals, or other relevant information.
- Record daily progress on obtaining current or missing orders, including the date, action taken, and follow-up date, and review the prior week’s activity.
- Support order escalation efforts by working with the home office, referral partner, and care coordinator.
- Join meetings to review outstanding face-to-face documentation, hand-carry items, orders, and escalations, and share plans for collecting missing paperwork.
- Print and organize orders for each care coordinator and review a weekly plan for obtaining signatures.
- Be ready to send outstanding orders, face-to-face documents, and hand-carry paperwork to a care coordinator if they are visiting a referral source and can get the signature during that visit.
- Run weekly reports for face-to-face items and orders, and work with care coordinators to make sure orders are completed within 14 days or within the timeline appropriate for the patient’s case.
- Review potential write-off accounts with RDS and help decide whether the account can be resolved or should be submitted for write-off.
- Communicate in a professional manner with all departments to resolve open orders.
- Respond quickly to questions and issues, aiming to resolve concerns and discrepancies without delay.
- Follow HIPAA requirements across all accounts and records.
Requirements
- You must have a high school diploma or an equivalent credential.
- Prior experience using an electronic medical record system is required.
- You should bring strong communication, organization, and time-management abilities.
- Advanced typing ability and solid computer skills, including Microsoft Office, are necessary.
- You need to be able to juggle several software platforms at once while protecting the accuracy and integrity of the data.
- You should work well in a deadline-focused environment.
- You must be able to stay productive when workload and deadlines are heavy.
