Role overview

The Patient Pathway Coordinator will sit within a central team and help keep the patient journey organised, joined-up, and focused on individual needs. The role supports a smooth administrative flow around each patient, while also providing cover for colleagues to maintain a dependable service at all times.

This post helps patients through their hospital experience so that care is delivered promptly, efficiently, and in coordination with the wider multidisciplinary team. A key part of the job is to oversee patient pathways safely and effectively, act as a consistent contact for appointment-related queries, and keep the patient and carer experience central throughout the pathway. The role also requires monitoring referral-to-treatment and cancer waiting time standards, raising any potential breaches to the appropriate manager, and working closely with MDT coordinators to ensure pathways are managed correctly.

In addition, the post holder will support data quality and validation work, confirm that pathway start dates are recorded for all patients, and assist the management team in identifying opportunities to improve pathway processes.
