The team works with community partners to design practical, person-centered solutions that support individual needs.
Role Summary
This full-time Case Coordinator role is part of Adjoin Veterans and supports Veterans in securing and keeping permanent housing through a housing-first approach. The position is based in San Diego, CA 92108, USA and serves Veteran families through programs supported by federal Veterans Affairs funding and state resources, including Supportive Services for Veteran Families (SSVF).
Key Responsibilities
The Case Coordinator partners with Veterans and internal teammates to help them find affordable housing and maintain stability. The role includes direct service delivery to Veterans and families who are experiencing homelessness or are at immediate risk, including individuals who may face mental health, disability, or substance use-related challenges. The position requires regular check-ins with each Veteran on the caseload every week.
The coordinator will create Housing Stability Plans and practical action steps aimed at achieving and sustaining permanent housing. A key part of the job is helping Veterans improve income levels to support long-term stability. The role also involves writing detailed case notes, entering documentation into HMIS (Clarity) every week, keeping referral logs current, handling paperwork accurately, and meeting all related deadlines.
In addition, the coordinator must maintain respectful and professional communication with Veterans, household members, and collateral contacts. Ongoing referrals and resource connections are expected throughout each participant’s time in the program, along with proper documentation. All required training must be completed within the assigned timelines.
The position also requires adherence to safety standards and may include transporting Veterans to appointments, housing searches, or other locations while maintaining safety and punctuality.
Compensation and Benefits

Requirements
Applicants should hold a bachelor’s degree, or have two years of equivalent experience and/or educational background in working with Veterans and/or people who have experienced homelessness or are at risk of homelessness. Candidates must be 18 years of age or older and able to pass a pre-employment drug screen and background check. Depending on assigned duties or compliance needs, physical evaluations may also be required, including a TB test and/or lift-and-transfer test.
A current certification in an approved First Aid and CPR course is required, or it must be completed within 45 days after hire. Candidates must be able to travel within the counties served and also travel across the state quarterly for meetings, trainings, and similar activities. An insured, registered vehicle in safe operating condition and a valid driver’s license are required. The employee must also complete all assigned Adjoin training programs.
