Role overview
This position sits within a multidisciplinary care team and focuses on delivering well-rounded patient support. The role combines patient engagement, care coordination, preventive health outreach, and the use of digital tools to help patients and caregivers take a more active role in managing health.
You will help improve awareness of available support services and guide patients toward timely actions that can strengthen health outcomes.
Key outcomes
- Develop strong working relationships with both internal care-team members and external stakeholders, including patients and caregivers.
- Support better clinical results through preventive screening efforts, with the aim of lowering readmissions and reducing escalations to tertiary care.
- Collect and use social determinants of health data that may affect social and medical outcomes.
- Use technology to improve patients’ understanding of health topics and their access to relevant resources.
Core duties
- Assist in coordinating patient care activities and documenting care delivery using IT systems.
- Track, complete, and record established clinical indicators and patient care activities accurately.
- Take basic patient measurements as part of screening and monitoring workflows.
- Help close gaps in preventive screening and vaccination under national programmes and initiatives.
- Provide simple health education on lifestyle management, social and physical activity, and self-care.
- Support social prescribing efforts and guide patients in using home monitoring devices such as blood pressure, glucose, and weight trackers.
- Advise patients on vaccination needs and related preventive care steps.
Requirements
- A diploma is preferred for this role.
- Training or qualifications in health promotion will be an added advantage.
- Prior experience in healthcare is preferred.
- Strong verbal and written communication abilities are needed.
- A pleasant manner and professional personality are important.
- Good interpersonal, coordination, and teamwork skills are expected.
- You should be proactive, positive, and genuinely interested in patient care.
- Ability to communicate in English and a local language or dialect is required.
- Comfort using Microsoft Office tools, including Excel, Word, and PowerPoint.
- Ability to work confidently with IT systems and programs is important.
Additional information
This role is based in Singapore and is a full-time, onsite position. It involves close collaboration with patients, caregivers, and healthcare colleagues, with a strong emphasis on preventive care, health education, and digital support in care delivery.
