Role overview
Leeds Community Healthcare NHS Trust is looking for an experienced staff nurse to join the West of Leeds Woodsley Neighbourhood Teams. This post sits within a community service model that supports the NHS long-term plan by delivering more care in patients’ homes, helping people leave hospital sooner, and reducing avoidable admissions.
What the role involves
- Delivering high-quality nursing care as part of a Neighbourhood Team.
- Working in a multidisciplinary setting to provide coordinated, holistic care.
- Collaborating with District Nurses, Community Matrons, occupational therapists, physiotherapists, Neighbourhood Clinical Assistants, Therapy Assistant Practitioners, and Self Management Facilitators.
- Supporting safe, compassionate, proactive care while helping colleagues and service users through change.
- Using case management skills and contributing to integrated community care delivery.
