Role overview
Hope For Tomorrow is hiring a full-time Recovery Case Manager for its residential setting in Point Pleasant, WV. This position focuses on supporting clients in recovery through consistent one-on-one contact, aftercare coordination, and communication with families and referral partners.

Key duties
- Meet clients regularly one-on-one and serve as the main point of contact for their family members, loved ones, and referral sources.
- Plan and carry out aftercare services tailored to each client’s needs.
- Support clients with outside needs such as court appearances, unemployment applications, and FMLA-related matters.
- Keep clients accountable for their behavior in the community and for completing assigned work.
- Prepare accurate documentation promptly and maintain records in the EMR.
- Track compliance with client contracts, phone rules, and buddy relationships, and ensure updates are recorded at shift change.
- Make weekly family calls, with additional communication when needed, and log all contact in the EMR.
- Discuss progress, behavior, contracts, development opportunities, programming, and aftercare during family outreach.
- Coordinate and schedule phone and in-person meetings with family members and loved ones.
- Contact family members within 24 hours of admission.
- Contact referral sources within 24 hours of admission and follow up with weekly calls.
- Meet with clients at least once per week to review treatment progress.
- Carry out step-downs and discharges as required.
- Upload or delegate current ROIs and aftercare plans into the EMR.
- Attend required meetings such as Morning Meeting, RCM Meeting, Supervision as needed, Safety Squad, and Quality Assurance.
- Support client engagement beyond an individual caseload when necessary.
- Introduce yourself to new clients on the caseload within 48 hours of admission and document the interaction in the EMR.
- Coordinate with all departments and oversee the client’s stay at Hope For Tomorrow.
- Create practical aftercare plans that fit the client’s real needs.
- Vet client visitors and phone calls.
Qualifications
- A bachelor’s degree in Human Services, Criminal Justice, or Psychology is required.
- Applicants should have extensive experience in the mental health and substance use disorder field.
- Strong time management skills and the ability to handle multiple projects at once are necessary.
- Candidates must be able to communicate clearly in written, electronic, and verbal formats.
- Sound judgment and the ability to identify workable alternatives are important.
- Successful applicants must meet and continue to meet pre-employment requirements, including reference checks, state and national background screening, a 10-panel urine drug screen, DMV check, insurance verification if applicable, and license or credential verification.
