Role Overview

This position is temporarily hybrid, with travel expectations outlined below. The official title for the role is Lead Care Manager (LCM). In this role, you will build tailored care plans, coordinate member services, partner with families and Primary Care Providers, and help members access the care and support they need. The work includes assessing needs, guiding next steps, resolving administrative barriers, and supporting members with compassion and professionalism.

The role serves members enrolled in the Enhanced Care Management program and may involve one or more of the populations listed in the job details.

Key Responsibilities

  • Speak with members to identify gaps in medical care and social needs, provide education about conditions and medications, and create individualized care plans.
  • Handle member questions and concerns while following hospital and legal requirements.
  • Work with interdisciplinary teams, identify medical and community resources, and help coordinate social service plans.
  • Keep regular contact with members through telehealth and in-person visits.
  • Advocate for members, work with healthcare providers, set up appointments and treatment plans, review progress, and help remove barriers to care.
  • Communicate with members and families in a compassionate, professional manner.
  • Support coordination related to behavioral health, substance use concerns, and community resources.
  • Take on other tasks as assigned.

Populations Served

  • People experiencing homelessness.
  • Individuals at risk of preventable emergency room or hospital use.
  • Adults with serious mental health needs and/or substance use disorder needs.
  • People transitioning from incarceration or who have been justice-involved within the past 12 months.
  • Adults in the community who are at risk of long-term care institutionalization.
  • Adult nursing facility residents moving back into the community.
  • Children and youth enrolled in California Children’s Services (CCS) or CCS Whole Child Model (WCM) with additional complex social needs.
  • Children and youth involved with child welfare.
  • Adults with intellectual or developmental disabilities who also meet eligibility for another adult ECM population.
  • Pregnant or postpartum individuals, including those in the Birth Equity population of focus.

Education and Experience

  • High school diploma or GED is required.
  • At least 1 year of experience in case management, member care, customer service, call center, or a related support role is required.
  • A valid California driver’s license and active vehicle insurance are required.
  • MA certification or familiarity with medical terminology is preferred.

Skills and Abilities

  • Strong verbal and written communication skills.
  • Good interpersonal and customer service ability.
  • Solid organizational skills.
  • Bilingual Spanish is preferred.
  • Comfort with computers for documentation, email, and chat support.
  • Ability to work independently and as part of a team.
  • Strong multitasking, prioritization, and attention to detail.
  • Resourcefulness and calm problem-solving under pressure.
  • Strong speaking, reading, typing, and proofreading skills.

Physical and Travel Requirements

  • You must be able to use your personal vehicle for outreach visits; mileage is reimbursed at the standard IRS rate.
  • Current tuberculosis testing and clearance must be maintained in line with company policy and applicable local and state health rules.
  • The job involves long periods of sitting and computer use.
  • You should be able to lift up to 15 lbs.
  • Most member concentration is in Stockton, CA, so comfort driving in that area is necessary.
  • The first week is based out of the San Ramon office.
  • For the first month, weekly Wednesday office visits are required.
  • After the first month, office visit frequency will be reviewed and may drop to once per month depending on business needs.
  • The commute to the San Ramon office counts as work time, and employees may begin their shift from home before traveling.

Compensation and Benefits

The pay range is $25 to $28 USD per hour.

Full-time employees become eligible for benefits after 60 days of employment. Benefits include medical, dental, and vision coverage; short- and long-term disability; voluntary accident, critical illness, and hospital indemnity plans; flexible spending accounts; an Employee Assistance Program; paid vacation and sick leave; paid holidays; an employee referral program; and a company discount program. The 401(k) plan becomes effective after 1 year of employment.
