Job description
Role Overview
The medical scribe supports the attending physician by preparing, updating, and refining office documentation. The role includes gathering medical records from outside providers before appointments for both new and returning patients, reviewing and scanning documents into the electronic health record, and helping ensure that information is accurately collected and shared with the care team. Most work is carried out in an outpatient clinic, with occasional inpatient consult support as needed.
Key Duties
- Obtain pertinent medical records from external organizations while following HIPAA-compliant procedures.
- Stay in contact with patients before, during, and after visits so the care team has complete and timely information.
- Draft, revise, and format reports in printed or electronic form for physician review, signature, correction, and inclusion in the medical record.
- Prepare medical reports, correspondence, patient-care documentation, statistics, research-related materials, and administrative documents.
- Proofread transcribed or dictated content for spelling, grammar, clarity, consistency, and correct medical language.
- Spot documentation errors and confirm the accurate details with physicians.
Qualifications
- A high school diploma or equivalent is required.
- Prior experience preparing, editing, and assembling medical documents with accurate vocabulary and grammar is needed.
- A college degree is preferred.
- Previous medical scribe training or work experience is preferred.
- Familiarity with computers and medical records software is preferred.
- Strong communication skills are required for interacting with healthcare professionals and patients about patient information and task assignments.
- The ability to maintain strict confidentiality is essential.

- A strong work ethic is important, including integrity, a positive attitude, organization, respect for leadership, good attendance, a willingness to keep learning, and comfort asking for help when needed.

Working Conditions and Physical Requirements
This position is based in a medical office and may also involve hospital settings when necessary. The environment may include moderate noise from medical equipment. The work involves prolonged standing and sitting, close visual attention to detail, listening and understanding spoken information, reading written material, and clearly communicating with others. Daily tasks require frequent use of the hands for typing, filing, and signing or writing documents. The position also requires the ability to lift at least 15 pounds.
