Role overview
This secondment opportunity is for a FEAT Sister within University Hospitals of Derby and Burton NHS Foundation Trust in Derby, England. The post focuses on coordinating the care and discharge process for frail older patients across MAU, ED, Wards 101, 306, 204 and fSDEC, working closely with the FEAT team to deliver comprehensive geriatric assessment.
What the role involves
You will help ensure that frail elderly patients receive high-quality clinical care and that discharge planning is managed effectively from admission through to transfer home or to an alternative setting that better suits their needs. The role requires a holistic approach, working with primary and secondary health and social care professionals to assess, plan, implement and review discharge care.
You will also support the early identification of frail older people on admission so they can be correctly flagged within the Trust, and you will help maintain safe, smooth patient flow through strong use of the electronic WhiteBoard and active coordination with the multidisciplinary team.
Key duties
Maintain clear communication with nursing and medical colleagues, and work in partnership with patients, relatives and carers in a way that respects dignity, courtesy, privacy and personal needs. Keep accurate and timely records of discharge assessments, interventions and contacts, and manage your workload effectively so priorities are handled within the FEAT team’s way of working.
All nursing or therapy procedures must be carried out in line with Derby Hospitals NHS Trust policies, with appropriate action taken when urgent or emergency situations arise.
