The role involves working across office, community and home-based settings, collaborating with a multidisciplinary team, and supporting children aged 0–9 years who have developmental delay and/or disability. You will work closely with families to understand needs, guide planning, and connect them with suitable services.
Key accountabilities

- Work alongside families to help children reach their potential through coordinated planning and support.
- Build and maintain strong relationships within the community to improve access and participation.
- Collaborate with professionals from different disciplines to provide coordinated assistance.
- Use empathy and sound judgement when managing challenging or sensitive family situations.
- Review developmental assessment information and use it to shape practical support decisions.
- Help families access early childhood support through the NDIA pathway.
- Carry out functional assessments for children experiencing developmental concerns or disability-related needs.
- Assist families with NDIS plan reassessments, including gathering evidence and developing goals.
- Link children and families with community and mainstream services that support participation and wellbeing.
