Role overview

This is a partner-company opportunity in which applications and later-stage hiring steps are handled by the employer's internal team. The role is for a Community Wellness Advocate in the United States.

You will act as a connection point between patients, healthcare professionals, and community-based resources, helping people with higher health risks move toward steadier health and better day-to-day well-being across both medical and social care environments. The work involves meeting patients in hospitals, homes, shelters, clinics, and other community settings to understand needs, earn trust, and shape care plans that address clinical concerns as well as housing, food, transportation, and behavioral health barriers. This is a field-centered, community-facing position that depends on empathy, cultural awareness, and strong coordination with interdisciplinary teams.

Key accountabilities

  • Reach out proactively to eligible patients in hospitals, clinics, homes, shelters, and community locations to explain services, complete intake screenings, and begin care-management engagement.
  • Create, apply, and track personalized care plans together with patients, families, and cross-functional care teams, keeping goals aligned with wellness and health outcomes.
  • Teach patients in ways that are culturally and linguistically appropriate so they can better understand their conditions and manage their own health.
  • Work with physicians, social workers, and community agencies to connect individuals with needed support and services.
  • Support patients as they move through the healthcare system by arranging appointments, helping with visit attendance when required, and assisting with medication access and follow-through.
  • Record patient interactions, care plans, and progress notes accurately and on time in care-management tools such as Epic.
  • Champion patient needs by spotting obstacles to care and helping secure housing support, food programs, transportation, financial help, behavioral health services, and other community resources.
  • Join case conferences, team huddles, training sessions, and outreach activities that strengthen program outcomes and ongoing improvement.

Requirements

  • High school diploma with relevant community experience, or a bachelor's degree in a related discipline.
  • At least 2 years of experience in healthcare, public health, social services, or community outreach.
  • Valid driver's license for field-based community work.
  • Solid understanding of healthcare systems and the social factors that influence health.
  • Strong relationship-building, listening, and communication skills across diverse groups.
  • Good organization and multitasking skills, with the ability to work both independently and as part of a team.
  • Comfort using Microsoft Office and data-entry or tracking systems.
  • Cultural awareness and sensitivity to how language, income, and culture affect health outcomes.
  • Haitian Creole or Spanish fluency is strongly preferred.
