Role overview
This permanent, full-time Case Manager position is based in Port Augusta, South Australia, with Calvary Home Care in the Flinders Far North region. The role focuses on coordinating a client caseload funded under CHSP and HCP, managing budgets, supporting the intake of new clients, and ensuring care arrangements continue to support each client’s independence.
What the role involves
- Oversee a caseload of home care clients funded through CHSP and HCP programs.
- Track and manage budgets across client care plans.
- Support the onboarding of new clients and help establish their care arrangements.
- Coordinate services to maintain client independence and quality of care.
- Work with stakeholders and internal teams to deliver effective support.
Key capabilities needed
- Strong time management and prioritisation skills.
- Working knowledge of Aged Care Standards.
- Confidence in liaising with internal networks and external stakeholders.
What you should bring
- Proven experience in a similar position.
- A tertiary qualification in the community sector is preferred.
- Background in aged care, community services, or disability support, especially with clients who have complex needs.
- A current driver’s licence and access to a reliable, registered, and insured vehicle.
- Ability to use a smartphone for work-related tasks.
- Familiarity with the My Aged Care portal and assessment process.
- Comfort working collaboratively within a team environment.

- NDIS clearance, or a willingness to obtain it.
- Police clearance issued within the last 6 months, or willingness to obtain one.
