About the role
SunCrest Companion Services, part of LHC Group and within the Optum family of businesses, supports hospitals, physicians, families, and communities across the country as a post-acute care partner. The organization focuses on helping people feel their best while building meaningful connections among patients, families, teammates, and the communities it serves.
In this position as a Home and Community Based Services Aide, you will help an individual remain safely in their own home by providing hands-on personal support, help with hygiene, and assistance with everyday household needs.
Key responsibilities
- Deliver personal care and help with routine daily activities such as bathing, grooming, dressing, walking or transferring, and medication reminders.
- Assist with household chores, meal preparation, and transportation support for appointments or errands when needed.
- Track and record client status, vital signs, and any incidents; protect confidentiality and use the EVV system accurately.
- Maintain a safe care environment, use medical equipment correctly, and respond quickly to client needs.

The hourly wage for full-time work is between $14.00 and $20.00, and all applicable minimum wage requirements will be followed.
Required qualifications
Applicants must have a valid driver’s license, vehicle insurance, and dependable transportation or access to public transit. A current CPR certification is also required. The role calls for flexibility in working hours and the ability to work in any home setting regardless of the client’s age, race, creed, color, sex, disability, or financial condition.
Preferred background
Experience in home care for 6 months or more is preferred.
