An opportunity has opened for an enthusiastic, self-motivated physiotherapist to join a cardiac and pulmonary rehabilitation service within a community-based NHS trust. The post involves supporting patients with cardiac and respiratory conditions through specialist assessment, treatment and rehabilitation in their homes and in community venues across Barnsley.

Main purpose of the role
You will work within a multidisciplinary cardiac and pulmonary rehabilitation team, contributing physiotherapy expertise and managing your own caseload with appropriate senior support when required. Work is service-based rather than venue-bound, so although you may be assigned a base, you should expect to move between locations during the week. Travel across the service using a vehicle is part of the role.
Key duties
Typical work includes triaging referrals, completing initial assessments and exercise testing, supervising patients during exercise, carrying out discharge assessments, providing education, and documenting patient information electronically. You will also contribute to the day-to-day handling of incoming group tasks.
The role includes assessing patients with a range of cardiac and respiratory conditions, planning treatment programmes, reviewing progress, adjusting interventions where needed, and offering guidance to patients, carers and other professionals involved in care.

You must maintain HCPC registration, keep your practice up to date through continuing professional development, and maintain accurate records in line with professional and Trust standards. Safe use of equipment, patient aids and appliances is essential, as is appropriate risk assessment for your caseload and working area.

Person specification
Applicants should bring therapy experience in a community setting, ideally with respiratory or cardiology patients, plus experience working in a multidisciplinary team. Strong physiotherapy reasoning, treatment and handling skills, good communication, organisation and record-keeping ability are required. Strong IT skills and presentation ability are desirable.
Essential qualifications include a BSc in Physiotherapy or an equivalent GradDipPhys qualification, together with HCPC registration. Membership of the MCSP is desirable.
Physical capability to perform the duties of the role is required, along with a satisfactory sickness record over the previous two years, subject to fair consideration where sickness relates to disability or pregnancy.
Personal attributes include the ability to work to deadlines, flexibility around working times and practices, including evening and weekend work where needed, and a current driving licence with access to a car during the working day, subject to reasonable adjustments where required.
