Role Overview

Chronic Care Staffing is looking for a compassionate, highly organized Remote Enrollment Coordinator with CMA/RMA credentials to help patients join Chronic Care Management (CCM) and Remote Patient Monitoring (RPM) programs. In this fully remote position, you will act as a dependable point of contact, explaining available support services and making sure enrollment and documentation steps meet CMS and HIPAA requirements.

This role is well suited to certified medical assistants who enjoy patient-facing conversations, educating people about healthcare services, and handling remote clinical coordination in a busy setting.

Work Schedule

The schedule runs Monday through Friday.

Compensation

The hourly pay range is $17 to $20.

Responsibilities

  • Reach out to patients to introduce CCM and RPM services and walk them through the value of each program.
  • Support patients through consent and enrollment steps from start to finish.
  • Record patient conversations and enrollment activity accurately in EMR platforms.
  • Follow CMS, HIPAA, and company documentation rules at all times.
  • Respond to questions about care coordination services and what patients should expect from the programs.
  • Work closely with providers and internal staff to keep communication and patient handoffs running smoothly.
  • Stay on top of daily outreach targets and follow-up tasks while keeping workflows organized.

Requirements

  • Active CMA or RMA certification recognized nationally.
  • Current Basic Life Support (BLS) certification.
