About Wellness Equity Alliance
Wellness Equity Alliance (WEA) is a nationwide, multidisciplinary health organization focused on delivering integrated, community-centered care for people most affected by health inequities. Its model combines mobile and field-based services to provide medical care, behavioral health support, substance use treatment, harm reduction, and care coordination in settings such as encampments, schools, reentry locations, rural areas, and tribal communities. Role Summary
The Patient Care Coordinator is an important part of the WEA care team and focuses entirely on remote outreach. The role involves making phone outreach to prospective CalAIM members, explaining available services, and helping eligible individuals enroll in Enhanced Care Management (ECM) and Community Supports programs. This is a telephonic, outreach-driven position that requires comfort with frequent outbound calls and a strong interest in improving outcomes for underserved communities.
Compensation and Work Setting
The pay range for this position is $25 to $28 per hour, with the final offer depending on experience, qualifications, and the scope of the role. California-based candidates are preferred, and the schedule requires work in the Pacific Time zone. The role is fully remote.
Key Responsibilities
In this role, you will contact prospective CalAIM members through outbound calls, explain WEA programs and services, assess eligibility and interest, and help move individuals into the right care pathway. You will also keep detailed records of outreach, patient conversations, and enrollment results, work toward daily productivity goals, and coordinate with care managers and other team members to ensure timely follow-up and smooth handoffs.
You will be expected to follow approved scripts, workflows, and compliance standards, maintain privacy and confidentiality, participate in meetings and training, and contribute to ongoing quality improvement efforts. Additional duties may be assigned as needed.
Documentation, Compliance, and Quality Standards
Accurate, real-time documentation is required for all patient interactions using approved systems and processes. The role also requires adherence to HIPAA, CalAIM, health plan contractual standards, and other privacy or regulatory requirements. Participation in quality assurance reviews and coaching sessions is part of the position.
Working Conditions
This is a fully remote position that involves long periods of phone work and computer use. The selected candidate must have a private workspace that meets HIPAA requirements and a reliable internet connection. Occasional participation in virtual meetings and trainings is expected.
Physical Requirements
The job involves extended sitting, continuous computer work, clear communication by phone and video, and repeated hand and wrist movements associated with keyboard and mouse use.
Requirements
Candidates should have at least a high school diploma or equivalent and a minimum of one year of experience in patient-centered care, patient services, healthcare outreach, call center operations, care coordination, or a related field. Strong verbal communication, active listening, organization, and documentation skills are necessary, along with the ability to work independently in a remote setting.
Applicants should be comfortable handling a large volume of outbound calls while staying professional and empathetic. Proficiency with Microsoft Office and web-based tools is required.
Preferred Experience
Prior experience supporting Medicaid or Medi-Cal populations is preferred, as is familiarity with CalAIM, Enhanced Care Management (ECM), Community Supports, or population health programs. Bilingual English/Spanish candidates are strongly preferred. Experience using EHR platforms, CRM tools, or care management systems is also beneficial.
