About the Role
CommWell Health is seeking a clinician to deliver primary care services in a rural North Carolina setting. The organization has supported local communities with patient-focused care for 50 years, and this role is centered on improving access, strengthening families, and contributing to a mission-driven healthcare team.
What You Will Do
- Deliver hands-on outpatient primary care, including assessment, diagnosis, treatment planning, and charting in line with established protocols.
- Evaluate patients, prescribe appropriate medications, and arrange laboratory work or imaging when clinically indicated.
- Oversee chronic condition management, encourage patients to meet self-care goals, and track their progress over time.
- Work closely with multidisciplinary care teams, take part in case discussions, and support others as a mentor or preceptor.
- Stay involved in continuing education, quality improvement activities, and initiatives tied to the patient-centered health home model.
Required Qualifications
- Current BLS certification from either the American Red Cross or the American Heart Association.
- Active, full authorization to practice medicine issued by the NC Board of Nursing or the NC Board of Medical Examiners, along with a valid license.
- National certification as an MD, DO, FNP, or PA.
Preferred Background
- At least one year of professional experience.
- Prior experience serving rural or underserved communities.
- Exposure to team-based care models or PCMH/PCHH frameworks.
