Role Overview
Rendr360 is the centralized support arm for the medical practices, handling services such as care management, care coordination, and continuous quality improvement. The team supports several patient-focused programs designed to maintain continuity of care between office visits and improve both clinical outcomes and patient satisfaction. One of those initiatives is the Transitional Care Management (TCM) Program.
The TCM Navigator supports patients as they move from extended care settings such as hospitals, rehabilitation centers, or nursing facilities into outpatient care. This person helps connect patients with Rendr360 nurses, arranges appointments, and supports the completion of quality measures.
Key Responsibilities
- Work with internal teams and outside partners to deliver a high-touch, concierge-style experience for patients who are newly entering the system or moving through it on their care journey.
- Support clinicians with scheduling, patient notifications, follow-up tasks, and other work tied to closing care gaps.
- Review and manage daily admission, discharge, and transfer data received from Health Information Exchange partners.
- Assist with quality improvement, care coordination, and population health initiatives across multiple stakeholders.
Qualifications
- Fluency in English plus Chinese (Mandarin or Cantonese) is mandatory.
- A college degree is preferred.
- Must be able to handle multiple tasks and work across several systems, including the EMR, to share medical information and provide patient support after visits.
- Strong commitment to excellent patient care and patient satisfaction.
- Professional, kind, and empathetic communication style.
- Good communication, people skills, and organizational ability.
- Previous experience in a medical office is an advantage but not essential.
- Basic familiarity with medical terminology is helpful but not required.
