Key responsibilities

• Support referrals by assessing client needs, arranging initial evaluations, and guiding families toward suitable care management support.
• Work in close coordination with internal teams to maintain continuity of care and ensure smooth communication during the client journey.
• Record referral activity, partnership development, and outreach efforts accurately in the CRM for reporting purposes.
• Work toward outreach and referral targets that contribute to overall business growth.

Requirements

• A current RN, LPN, or social work license is preferred, though comparable clinical experience may also be considered.
• Background in clinical outreach, case management, sales liaison work, or a similar healthcare-facing role.
• Strong relationship-building, communication, and trust-building skills across varied professional audiences.
• Ability to work independently, stay organized, and manage several priorities at once.
• Working knowledge of healthcare systems, senior services, and local community support resources.
• Valid driver’s license and dependable transportation for regular travel within the assigned area.
