About Care Continuity
Care Continuity is changing how patient navigation works by blending clinical know-how, AI-powered insights, and human-centered support so patients can access the right care at the right time and place. Its platform helps health systems and care providers close care gaps, lower readmissions, and improve return on investment through more connected navigation workflows.
The company is building new ways to improve healthcare decision-making and is still expanding its impact.
Role Overview
In this position, you will support healthcare organizations in getting more value from their clinical and operational data. The initial focus will be on one of Care Continuity’s largest health system partners, where you will study patient movement and care utilization patterns using Health Information Exchange (HIE), ADT, referral, and clinical datasets.
Your analysis will help uncover ways to improve patient navigation, reinforce network integrity, reduce leakage, and support growth for both Care Continuity and its customers. The role brings together healthcare analytics, data investigation, client collaboration, and business insight generation. You will partner with product, analytics engineering, customer success, and health system stakeholders to turn complex data into practical recommendations and clear narratives.
The ideal candidate is deeply analytical, comfortable working with imperfect healthcare data, and naturally curious about how data can improve care delivery and patient outcomes.
Key Responsibilities
- Review ADT, CCD, and other healthcare data sources to identify patterns, operational opportunities, and actionable insights.
- Create and maintain an up-to-date inventory of data feeds, available fields, and facility coverage, while spotting gaps, unused data, and duplicate or missed integration work.
- Examine data quality issues and work with internal teams and customer stakeholders to improve completeness, accuracy, and usability.
- Run exploratory analyses to find opportunities in patient navigation, care coordination, network integrity, referral management, and patient retention.
- Prepare reports, dashboards, and presentations that present findings clearly to healthcare executives and operational teams.
- Assist with customer implementations by checking incoming data and supporting data mapping efforts.
- Collaborate with analytics and data engineers to improve BI data pipelines and take ownership of the reporting-layer datasets and semantic models built on top of them.
- Manage the full process of moving validated data into the BI/reporting environment by defining data models, building datasets, and keeping published reports accurate as source feeds change.
- Investigate interface and data flow problems across ADT, CCD, and other feeds.
- Work with customer success and product teams to turn analytical findings into recommended actions.
- Help measure business impact and identify ROI opportunities for health system customers.
- Contribute to the creation of new analytics products, dashboards, and insight-generation capabilities.
- Document data assumptions, business rules, and the methods used in analysis.
Requirements
- Between 3 and 7 years of experience in healthcare analytics, healthcare data management, population health analytics, clinical informatics, or a similar area.
- Advanced SQL skills and the ability to work independently with large datasets.
- Experience working with healthcare data such as ADT, HIE, claims, referral, clinical, or encounter records.
- Practical knowledge of healthcare interoperability standards, including HL7, ADT feeds, and CCDs; FHIR is a plus. You should be able to interpret message structures, troubleshoot interface problems, and verify that incoming data maps correctly.
- Ability to investigate and resolve data quality, mapping, and interface issues.
- Strong analytical and problem-solving ability with careful attention to detail.
- Ability to explain complex findings to both technical and non-technical audiences.
- Experience creating dashboards, reports, and executive-level presentations.
- Comfort working directly with healthcare stakeholders and customers.
- A curious, proactive mindset and interest in using data to improve healthcare delivery.
- Strong Power BI experience, including building and maintaining datasets and data models, not just visuals, and publishing reports that remain accurate when underlying feeds change.
- 3–7 years of experience in healthcare analytics or a related discipline is preferred for this role.
Preferred Experience
- Experience with implementing or supporting Health Information Exchanges (HIEs).
- Background in provider network analysis, referral analytics, care coordination, or patient navigation programs.
- Exposure to health systems, physician groups, or value-based care organizations.
- Experience in customer-facing analytics work.
- Work experience in health tech, population health, care management, or digital health.
Salary and Benefits
- Annual salary range: $100,000 to $130,000, depending on experience and location.
- Comprehensive benefits including medical, dental, vision, and a 401(k).
- Equity opportunities.
- Flexible PTO and a fully remote setup.
